Potomac Community Services offers the following services for Carroll County residents: Counseling Services, Targeted Case Management, and Mental Health Care Coordination. 


Our Counseling Services provides a supportive space to reach your mental health goals. We work with individuals, couples, families, children and adolescents.
 

 

Our adult Targeted Case Management (TCM) program offers mental health case management to individuals with mental health needs. This program aims to assist through advocacy, education, connection to community resources, and support maintaining health care treatment to make sure you experience a better quality of life. Once you know what you want to accomplish, your case manager will connect you to helpful community resources, attend medical appointments with you to help you understand and do what the doctor says, connect you to other services to assist with your basic needs, secure housing, and apply for benefits. 

 

Our Mental Health Care Coordination (MHCC) program aids children and youth up to age 18 who have mental health needs. Care Coordinators meet with children, families, and their support network in the community, in school, or at home to best meet the child and family's needs. The Care Coordinator works with the child and family to set and achieve their own goals. MHCC focuses on addressing mental health, physical health, education, finances, and socialization to create stability and promote healthy growth and development while the child and families gain independence. Care Coordinators help families apply for benefits, secure stable housing, communicate with schools, connect to community-based resources, and provide transportation to appointments when needed. High-intensity services are available for children and youth who have been hospitalized or placed out of the home. 

 

TCM Plus Services are available to children who meet the above criteria but have private insurance. 

 

TCM & MHCC services are covered by Maryland Medicaid. Please call our office with specific insurance questions for Counseling services.

The Partnership for a Healthier Carroll County, Inc., an affiliate of Carroll Hospital and the Carroll County Health Department, is a private nonprofit organization working to improve health by connecting people, inspiring action, and strengthening community.

Our organization is the coordinating hub that connects agencies and individuals in pursuit of a healthier community. It is often said that collaboration by many is what makes our community like no other. The Partnership is proud to be part of this ongoing tradition of teamwork and collaboration.

  • Track emerging health issues,
  • Influence policies that impact health,
  • Oversee community health assessments, and
  • Monitor health data to regularly “check the pulse” of our community.
